Are you a foster youth or probation youth between the ages of 18 -21 and you are homeless or at risk of being homeless? Do you know what it is like to live on your own in a safe and supportive environment and receive mental health services? Well, you might be interested in the Department of Mental Health contracted transitional housing program. This program will provide you with an opportunity to live in an apartment in the community while getting support from a team of mental health professionals. You will learn the skills to become self-sufficient by the time you exit this program. Your DCFS and/or Probation case will have to be closed prior to you entering this housing program.
For more information, check the FACT Sheet then talk to your CSW or ILP/Transition Coordinator.
